How long after starting your engine when completely cold before you start driving?
I've noticed on my Black '03 (wife usually drives the Silver '04) it can be very jumpy when cold. I'm starting to let it run longer, say about 5 mins or so, before pulling out of the driveway.
Don't know if the fuel pump is on its way out. When I changed the fuel filter at 95K it was filled with crud.

If the engine is or was recently at temp I will wait just long enough to put the seat belt on. If it isn't at temp I will wait until it idles down and then keep it under 3k RPM until it's at op temp.

When I start mine the rpm's flash up for about 30 seconds or so. As soon as they drop, I drive. If I don't wait it kills the engine when I put it in gear. Otherwise I'd start driving it as soon as it starts.

I start the car, then put on my seat belt, chek for any seat/mirror adjustments, etc, so maybe 45 sec to a minute.
I start it, pick the radio station or pick the CD and by then the RPM's have dropped to a normal idle then I drive off. I wait until it's at normal operation temp for WOT

Yep, what ^^^ they ^^^ said.
I take off immediately.
You say that knowing that someone will soon call you about replacing their motor.
Seriously, turn key to on, step on gas, WOT. In short order your engine will be warmed up.

That right there is the one and only answer ^
Anything else, and your pushing it.

I wait till the air bag light and everything else goes out and the seat belt is on then I am moving.
I always first start driving with the radio off to see if there is any sounds I havn't heard before then turn the
radio on and relax.

I wait in the winter, but during this time of year I usually start it and go. I wait until the temp is in the middle before I give it WOT, but I don't do that a lot here in the city.
If it decides to smoke that day, I wait till the smoke cloud fades away. If not, I turn off traction control, turn off overdrive, turn on CD player, taxi down the runway and depart.

Replaced EGR valve last week. It was sticking open. Rough idling and surging both gone.

I agree/- contact Ground control -taxi down to the corner -then contact Departure control and ROLL OUT
What I noticed was that the fuel filter was full of crud @ 95K. Do you mean to say the fuel filter hasn't been changed for 95 K miles or that the 95 K was the scheduled fuel filter change point? It's a 15K mile maintenance requirement.
Bad gas station tanks can introduce crud into the cars fuel system and really plug up a fuel filter. Happened to me once.
